Community Health Plan of Washington

Community Health Plan of Washington (CHPW) Apple Health Plan

Community Health Plan of Washington (CHPW) was established in 1992 as a Managed Care Organization (MCO) and rejoined our region in 2021 (a year after other MCO’s in WA). CHPW is the only Apple Health (Medicaid) insurance provider, not for profit, founded by Community Health Centers with a focus on health equity and antiracism.

WHAT? Care Management facilitates connection and access of services that are going to be the best for you and your needs. It is a collaborative process usually provided by social workers or nurses.

HOW? Care Management helps people find healthcare providers and schedule appointments to get care. People can email Care Management or the Community Health Worker. An alternative to connect with individuals is to call the customer service line.

ACCESS CHALLENGES? The CHPW community health care worker in our region knows the local clinics and can even be in person to help you. You can refer yourself or ask a case worker for this extra support. They can help you fill out forms to organize transportation for appointments and teach new skills to manage your own care while in your home.
