SYNC Request

SYNC accepts referrals from all sources, including families, schools, treatment agencies, for children youth and their families living in the 3-county region. Priority is given to referrals to youth in emergency departments or other non-treatment facilities.

The Release of Information should be signed by the legal parent/guardian of the youth if under 13, and youth over 13 will need to sign the release of information. If you have questions or concerns about signing the release, please contact SYNC staff for assistance.
