Community Health Plan of Washington (CHPW) was established in 1992 as a Managed Care Organization (MCO) and rejoined our region in 2021 (a year after other MCO’s in WA). CHPW is the only Apple Health (Medicaid) insurance provider, not for profit, founded by Community Health Centers with a focus on health equity and antiracism.
The Jefferson Teen Center (Tri-Area Teen Center) is a free and inviting place for 7th-12th graders. Located just behind Chimacum High School (portable building next to the football field), youth may come during hours of operation to hang out in a prosocial environment, The mission is to empower tri-area teens through socialization, enrichment activities, resources, and mentor support in an inclusive, safe environment.

Bremerton - WISe Kitsap County | CCS Family Behavioral Health WISe teams offer flexible support for youth and families with complex behavioral health needs. The team includes Parent/Caregiver Peer Counselors and Youth Peer Counselors.
PBH Youth Outpatient SUD program is client-driven and serves youth ages 13-18 years. Self-referrals and guardian referrals are welcomed. The program takes walk ins 2 days a week (currently Tues/Wed). Progress is necessary, but perfection is not. Clients have up to nine 9 hours of services per week across a flexible schedule between early morning until 6pm.
The Salish Regional FYSPRT program supports and encourages individuals to share their voices in a collaborative effort toward improving youth focused Behavioral Health systems of care.

Enhanced Case Management (ECMP) is a program within the Developmental Disabilities Administration (DDA). ECMP case managers have a smaller case load (1:30) than the standard caseload (1:75). For Individuals and families who need extra support when there are concerns about isolation, the home environment or quality of care.
